Assumption of Risk

I, the undersigned, understand that participation in activities, events, programs, or workshops hosted or sponsored by the Chelsea Chamber of Commerce (“the Organization”) involves physical activity and inherent risks, which may include, but are not limited to, falls, collisions, property damage, or other injuries. I voluntarily assume all risks associated with my participation.

Release and Waiver of Liability

In consideration of being allowed to participate in activities or events organized by the Chelsea Chamber of Commerce, I, for myself, my heirs, executors, administrators, and assigns, hereby release, waive, and discharge the Chelsea Chamber of Commerce, its officers, employees, volunteers, and event sponsors from any and all claims, liabilities, or damages arising from my participation, including claims arising from negligence, except for intentional misconduct or gross negligence.

Medical Authorization

I consent to receive medical treatment deemed necessary in the event of injury, accident, or illness during any activity or event. I acknowledge that I am solely responsible for all medical expenses incurred.

Photo/Media Release (Optional)

I grant the Chelsea Chamber of Commerce the right to photograph, record, or capture my image or voice during activities or events and to use such materials for promotional purposes, including social media, newsletters, and marketing, without compensation.
